Started with Revit 2023 Dyn 2.13 and had the exact same issue.
The nodes were coming lumped together, all grouping were lost, any files “Saved as” afterwards will not open as well showing UNKNOWN ERROR OPENING FILE
Finally the issue was caused by a node in error state, specifically Code Block in Error State (because I had a full sentence written inside a code block not as a string).
I deleted this error state code-block by opening in Sandbox, saved the file, reponed in DFR 2.13, everything was working OK. All node arrangement and groups were as it was supposed to be!
