As we don’t have a tag for Civil 3D or Revit, or an understanding of what a “Note Label Style with orientation of the reference alignment direction” means all we can do is guess.
Best to read this topic closely, and post a complete dataset and clear ask as to what the question is. if you cannot post files here, utilize a 3rd party service such as Dropbox, OneDrive, Box, Google Drive, etc…
